As owner and/or director, I sign below and acknowledge receipt of Westport Harbour’s Terms and Conditions and the accompanying three schedules (“Schedule1” – this application form,” Schedule 2” – information pack, “Schedule 3” – current fees schedule) and agree to be bound by such Terms and Conditions and the schedules therein. I also extend a personal guarantee that due and punctual payment will be made to you for services provided as per the terms and conditions. *

I acknowledge that if a Health & Safety Management System for my vessel(s) while in port has not been provided to Westport Harbour that my skipper and crew will abide by all H&S legislation and any site specific requirements as outlined by the port authorities. *

I confirm that I am either the owner of the vessel stated above or, as a company director, have the authority on behalf of the owner to enter into such Terms and Conditions. *

As skipper, I acknowledge receipt of Westport Harbour Limited’s information pack and will ensure this is kept on board the vessel and the crew are aware of its contents. *
